Which Insurances does Body Logic Physical Therapy Accept?

We accept Medicare and most Medicare supplement plans; this includes but is not limited to: Blue Shield, Anthem Blue Cross, AARP/United Healthcare, GEHA, Humana, CHAMPVA, Tricare, Mutual of Omaha, & Healthnet.

Do I need a doctor’s referral?

It is our office policy to have a referral if you would like to use your health insurance. This is especially important if you are a Medicare patient.

Cash Pay patients do not need a referral and can call (530)-272-7306 to schedule. Initial evaluations are an hour long at $150 and follow-ups are half an hour at $100 each.
Auto Accidents
I was in a motor vehicle accident and I have an open claim. Does my health insurance cover it and how do I schedule an appointment?
The aftermath of a car accident can be traumatic, physically and emotionally. Our physical therapists can treat a variety of injuries including whiplash, lower back pain, and fractures due to the accident. If you or a loved one are seeking treatment for any symptom related to your car accident, we can treat you, but the billing may be affected if you have an open claim or litigation.
If you have an open auto insurance claim, your health insurance is no longer responsible for it and we cannot bill them. We also do not do medical liens, wherein you are treated now and your settlement reimburses the facility at a later date.

​However, you can be seen as a Cash Pay patient. You pay up front and at the end of treatment you can receive an itemized superbill and list of all of your appointments to submit for reimbursement. You can call (530)-272-7306 and schedule an appointment at any time-- no referral needed.

Can Physical Therapy help me?

Physical therapy is designed around your musculoskeletal needs. Is your goal to get rid of pain, move around easier, or feel more steady on your feet? Do you want to get back to your normal day-to-day after surgery or an accident? Your physical therapist can create treatment plans that are individualized to your needs and lifestyle.
What kinds of treatment does Body Logic offer?

Your physical therapist may prescribe beneficial exercises or stretches tailored to your specific case. They can guide and perform them with you in your appointment and give you printouts that you can take home.

Certain cases might benefit from other forms of treatment, or modalities, such as ultrasound, E-stim, or ice.
What can I expect for my first appointment?

Once the paperwork is done*, either electronically or in office, your one-on-one appointment with the physical therapist will begin. They will perform a thorough evaluation, offer treatment, and they may set you up with a home program specific to your case. At the end of your session you will come out and schedule follow-up appointments at the front desk.​

*We send electronic intake forms by email shortly after we schedule your appointment; you can expect to spend approximately 15-20 minutes on it and it must be completed in one sitting. If you have any issues please call (530)-272-7306.
What should I bring/wear to my first appointment?

Please bring a photo ID and your insurance card(s) to your first appointment. You can also bring a copy of your medication list if you have one.

Wear clothing that is comfortable and easy to move around in. Your physical therapist may also want to examine the area, so something loose-fitting may be appropriate.
Do I need to have imaging done before making an appointment?

Not necessarily, but follow up with your doctor if you seek guidance on when to start PT. In some cases, you can start before getting imaging done and the physical therapist may even help you determine if and what kind of imaging would be beneficial.
I have had imaging done, should I bring the reports or CDs?

We cannot use the CDs directly, but if you had an MRI, X-ray, etc., you can bring your imaging reports for the physical therapist to see. If you don’t have your reports on hand, you can also call your doctor and have them fax the documents to us at (530)-272-7316.
